About Us
Rooted Dental Care started with the idea of creating a modern neighborhood dental office that is accessible to patients of all ages, in the heart of the Lincoln Park neighborhood in Chicago. We offer an uncompromising approach to dental care, utilizing advanced technologies, and foster long-lasting relationships rooted in our community. By providing education on prevention and general wellbeing, we hope to reduce the stigma around and alleviate the anxiety associated with dentistry.
Our policy is to have open and honest communication between all members of the Rooted team and our patients. Allowing our patients access to evidence-based information with consideration to their wants and needs, allowing them to have a voice in the care they receive.
We are dedicated to eco-dentistry and to becoming a more sustainable business in our community by:
- Using bamboo and recycled material products
- Separating amalgam waste from the water system
- Reducing plastic waste

Dr. Priyanka Patel
After graduating from Case Western School of Dental Medicine in 2011, Dr. Priyanka Patel, DMD, completed a one-year residency program at the Cleveland VA Hospital. This advanced training solidified her deep conviction and commitment to providing state-of-the-art dental care needs.
Being from Chicago, she could not think of a better city to come back to and establish herself. She enjoys taking the time with my patients to understand their needs and help achieve the final result they are looking for, whether it be maintaining or enhancing their smile. Dr. Patel focuses on helping reduce patient’s anxiety from the moment they step in the door to the moment they leave. Dr. Patel enjoys spending time with her husband, daughter Maya, and two dogs exploring the city in her free time. She also takes time to unwind both at home, painting or cooking, or abroad, exploring other countries and cultures.
Dr. Sara Imtiaz
Dentist
Dr. Sara Imtiaz grew up in the UAE and obtained her Bachelor’s in Dental Surgery from Pakistan. She moved to the USA in 2016 and received her Doctorate in Dental Medicine from UIC College of Dentistry.
She enjoys creating beautiful and functional smiles for her patients while treating each patient with a holistic approach. She believes in the power of empathy and prioritizes making her patients feel comfortable during their dental visits.
Outside of dentistry, Dr. Imtiaz is well-traveled and loves to continue to travel and learn about different cultures. During her downtime, she loves to run, explore Chicago on foot, and play video games.
Raegan Hop
Registered Dental Hygienist
Raegan graduated from Ferris State University with an Associate of Applied Science in Dental Hygiene and a Bachelor of Science in Dental Hygiene.
She enjoys building relationships with patients and making them feel comfortable when receiving dental care.
Being from Kankakee, Illinois, she visited Chicago often before moving here. In her free time, Raegan enjoys spending time with her cats, friends, and family, as well as going shopping and finding new restaurants and coffee shops to check out!
Samantha Howell
Registered Dental Hygienist
Sam graduated from Michigan State Univeristy with a Bachelors of Science in Biology and continued her education at University of Detroit Mercy, earning a second Bachelors of Science in Dental Hygiene.
She loves educating and encouraging her patients to make dental visits pleasant and put patients at ease.
Since moving to Chicago from Grand Rapids, Michigan, Sam has loved exploring the city and trying new food & drinks. She also loves going out into nature, hiking, and is scuba certified!
Accepted Insurance
At Rooted, we make the billing process as stress-free as we can. By verifying benefits prior to your visit, we are able to be transparent about your coverage. We provide estimates for the dental work you might need based on the information we receive about your policy. Rooted accepts all PPO insurances and those that do not have any insurance coverage.
Billing Procedures
We accept all major credit cards, check, as well as Care Credit (third- party financing). We also offer in-house payment plans.
